Recent studies have found that with the implementation of a Patient’s Medical Home, primary care practices have experienced several improved outcomes. Many changes occurred in the early implementation stages and were sustained and increased over time. For many outcomes, the greater the extent of Patient’s Medical Home implementation or stage of maturity, the better the results (e.g., on emergency room visits, quality, cost savings, prevention).
